Perimenopausal Menstrual Problem

As the women approach the age of 40 few variations in the menstrual pattern are expected. The ovarian reserve is determined for every women at her birth. Half of this reserve is lost even before the age of menarche when the ovulation begins. Along with the ovulating follicles, a few other follicles are lost in every menstrual period. When she reaches beyond 40-45 years the follicles reserve and quality of follicles also drop. As a result, ovulation becomes irregular and so do the periods.

But one must know alarming signals at this age-

  • Heavy flow
  • Flow with clots
  • Sudden change in the pattern of cycle or flow
  • Irregular flow with no cyclicity

Common reasons at this age:

  • Hormonal Imbalance
  • It can be corrected easily with medications
  • Endometrial hyperplasia
  • This can be precancerous
  • Reversible only till 1st stage
  • Stage 2 & 3 will require operative management
  • Uterine body/ Endometrial cancer
  • It should always be ruled out before any conservative management
